Medical disclaimer
Nothing on this Site creates a doctor-patient relationship between you and Optimal Spine Chiropractic. A doctor-patient relationship is established only after you have been formally accepted as a patient by a licensed chiropractor at our practice and following a proper in-person examination.
If you are experiencing a medical emergency, call 911 or your local emergency services immediately. Do not delay seeking emergency care because of information you read on this website.
Reliance on any information provided by this Site is solely at your own risk. Individual results from chiropractic care vary and are not guaranteed. Testimonials and patient stories on this Site reflect individual experiences and are not representative of every patient's outcome.
Appointments & cancellations
Scheduling an appointment through our website or by phone constitutes a request for services, not a guarantee of a specific appointment time. All appointments are subject to availability and confirmation by our office.
We ask that patients provide at least 24 hours' notice when canceling or rescheduling an appointment. Repeated late cancellations or no-shows may result in a cancellation fee or refusal of future scheduling.
Fees for services are due at the time of your visit. We accept most major insurance plans; however, it is your responsibility to verify coverage with your insurance provider prior to your appointment. We are not responsible for any charges your insurance plan does not cover.
